Simple Billing Option

When completing the Patient Billing Form included in your Tempus kit, we recommend selecting the following option in Section 2:

✔  “If my out-of-pocket cost with insurance exceeds $295, please automatically switch me to the self-pay price of $295.”

✔  Checking this box helps avoid unexpected costs and greatly speeds up your test completion time.

 

No Surprises Guarantee

Tempus is committed to cost transparency. Every patient who chooses to bill their insurance will receive a benefit check, and:

✔ No patient will pay more than $295 out-of-pocket

✔ Many patients pay less—or nothing at all​

✔ You may have no out-of-pocket cost if you have: Medicare Part B (outpatient services), Medicaid or Tricare

✔ You may be​ eligible for the Tempus Financial Assistance Program (FAP)

✔ If your cost exceeds $295… Tempus’ Customer Success Team will contact you before moving forward with your test to discuss options—but only if you did not check the $295 max box mentioned above. They may reach out via phone, email, or text.
