Center for Personal Wellness is an insurance friendly practice and can help clients utilize both their in-network and out-of-network insurance.

In-Network Benefits: For clients using one of the insurances below, we accept the contracted rate for that insurance. We are happy to help you verify your benefits and will file claims on your behalf.

We are in-network with the following insurance/EAP companies:

* Behavioral Health System (BHS) - services Crown, Midmark & Talbert House; Ulliance EAP - services OPUS Packaging

Out of Network Benefits: Your counseling services may be eligible for reimbursement through out-of-network benefits. We have partnered with Mentaya to help our clients save money on therapy by using their out of network insurance benefits. If you have out-of-network benefits, Mentaya will file claims and handle the insurance paperwork to make sure you get reimbursed. They charge a 5% fee per claim and have helped people get thousands of dollars back per year; however we absorb that 5% and cover it for our clients.

Use this tool below to see if you qualify for reimbursement for our services for any insurance we are not in network with. * This will only work with out-of-network coverage. Please refer to the section above for insurances we are IN NETWORK with and to have us check your IN NETWORK benefits.

Note:  Mentaya's goal is to save you time and money. It's completely optional, and as your therapist we do not benefit in any way from your participation.

** Please note, we cannot bill Medicaid, HMO or EPO plans out of network.

How often will I come?

Consistency brings the best results.

Most counseling clients come weekly and it's best to schedule a set time each week. Because of the importance of consistency and due to the fact that our therapist's schedules fill up fast, our Client Care Coordinator will set up the first 3 months of appointments. (*this can always change).

Committing to and prioritizing that time is ideal and equates to a better outcome. Occasionally, people attend therapy more often and may come twice a week for a period of time.  Others may have longer less frequent sessions. Many reduce counseling frequency once things improve and attend every other week then monthly to maintain gains. This will be determined with your therapist.

Cancellation Policy

We prioritize our clients by holding space for each person we see. When clients cancel an appointment, our clinicians offer that space to other clients who need an appointment that week. When clients cancel within 24-hours or no show, other clients miss out on an opportunity to be seen. You may find that you need to cancel an appointment due to unforeseen circumstances.  If you need to cancel an appointment, please notify us as soon as possible so that we may offer the time to someone who may be waiting for an appointment.

Anyone canceling an appointment with less than a 24-hour notice will be charged a $100 fee.  Insurance does not cover no shows and late cancellations, so is the client’s responsibility to pay. We will charge the credit card on file for no shows and cancellations within 24-hours of scheduled session.

How do I get started?

The quickest way to get support is by setting up a call using the link below. You will talk with Vanessa, our Client Care Coordinator who can answer your questions and match you with the best therapist.

We know it's not easy to seek counseling so we do our best to make the process user friendly.

Here's 2 additional ways to connect with us:

  • CPW is a paperless practice. After scheduling your appointment, our client care coordinator will set you up in our online portal.

  • Through our secure and HIPPA compliant portal, you will complete all of your intake paperwork and have access to billing. This must be completed 48 hours prior to your first appointment. This will allow your therapist to spend more time with you and less on paperwork and will additionally allow us to verify your insurance benefits if you are choosing to utilize them.

Please fill out the intake forms at your earliest appointment. Your appointment time will not be final until your intake paperwork is completed. Sessions cannot start without intake forms being completed.

You may call us at (419) 628-6156 and speak to our client care coordinator

Monday - Thursday 8:30am - 3:00pm or email us at any time.

Please note, we are not able to schedule appointments by walking into the office,

as we do not have front desk staff.
