* Couples therapy is not covered by insurance. There are limitations of what insurance policies cover with regards to couple’s therapy. For a detailed explanation, see the “Limitations of Insurance and Couples Therapy”.
Payments are due at the time of service and are made at the beginning of each session. Your therapist will discuss payment and explain the payment process in your initial session.
The Center for Personal Wellness is able to accept the following forms of payment: cash, checks, all major credit cards, HSA (Health Savings Accounts) and FSA (Flexible Spending Account) cards.
For clients using one of the insurances below, we accept the contracted rate for that insurance.
We are in-network with the following insurances:
Medical Mutual PPO
Anthem BCBS PPO
Behavioral Health System (BHS)
For all other insurance companies, we are an out-of-network provider. If you are eligible for out-of- network benefits (which we find that most people with PPO and POS plans indeed are), you may be able to be reimbursed a percentage for our services from your health insurance company. ** Please note, we cannot bill Medicare or Medicaid out of network.
As a courtesy, we can bill most out-of-network insurance companies directly on your behalf. Your counseling services may be eligible for reimbursement through out-of-network benefits, medical spending or health care savings accounts. Health insurance plans and benefits vary. If you are interested in using your health insurance, please call your insurance provider to inquire about reimbursement for out-of-network counseling services. CPW can also provide you with a receipt at the end of each month (called a SuperBill) which you can submit to your insurance company for out-of-network coverage/reimbursement.
We reserve space for each client we see. You may find that you need to cancel an appointment due to unforeseen circumstances. If you need to cancel an appointment, please notify us as soon as possible so that we may offer the time to someone who may be waiting for an appointment.
Anyone canceling an appointment with less than a 24-hour notice will be charged the full session fee. Insurance does not cover no shows and late cancellations, so is the client’s responsibility to pay. We will charge the credit card on file for no shows and cancellations within 24-hours of scheduled session.

CPW is a paperless practice. After scheduling your appointment, our client care coordinator will set you up in our online portal. Through our secure and HIPPA compliant portal, you will complete all of your intake paperwork and have access to billing. This must be completed prior to your first appointment. This will allow your therapist to spend more time with you and less on paperwork and will additionally allow us to verify your insurance benefits if you are choosing to utilize them.
Please fill out the intake forms at your earliest appointment. Your appointment time will not be final until your intake paperwork is completed. Sessions cannot start without intake form being completed.
You may call us at (419) 628-6156 and speak to our client care coordinator Monday, Tuesday or Thursday 10am-2pm or email us at any time. Please note, we are not able to schedule appointments by walking into the office, as we do not have front desk staff. Thank you!
